Create your Advance Directive
PURPOSE: To communicate your end-of-life wishes when you are unable to communicate yourself. You will file this form with the National Health System so that it is accessible to all providers. Rentev forms are available to download here in both Portuguese and English*.
When complete:
1. Send an email to balcao.rentev@ulsa.min-saude.pt to get an appointment to submit your completed form to SNS.
2. Go to the scheduled appointment with your forms plus your citizen’s card, residency card, or Crue.
3. You will need to renew your Advance Directive every 5 years. Of course, you may update or change your information at any time.

* The English version is provided to help non-portuguese speakers complete the form. However, you will need to transfer your answers to the Portuguese form in order for it to be accepted by SNS.
