What you can do
You can get prepared today
We invite you to first download and complete an Emergency Checklist and make it available in your home in case of an accident or medical issue at home. We also recommend setting up a QR code on your mobile in case of an emergency when you are not at home.
Next, we advise you to download and complete your Advance Directive and submit it to the SNS system. In that way all medical and emergency personnel will know your medical care preferences and contacts in the event you are unable to speak for yourself.
We provide workshops to assist you with completing your Advance Directive should you feel you need more information and assistance with this form and process.
You can organize non-medical support using these tools
Regarding the non-medical support tools we use, we offer them publicly for you to use as needed within your family and with your friends if that is your preference. They are immediately available to you. Additional tools are forthcoming. Be sure to stay informed by signing up for our monthly News Brief.
If you need additional assistance using or understanding these tools, please contact us.
